Notes:

1. The information collected in this form will be used by the Po Leung Kuk Celine Ho Yam Tong College Alumni Association (hereinafter referred to as "the Association" or "Alumni Association") for identity verification and the implementation of Association affairs. Depending on operational needs, the information collected may be shared with Po Leung Kuk Celine Ho Yam Tong College (hereinafter referred to as "the School") and other organizations. Apart from this, all information will be kept strictly confidential.
2. Fields marked with "*" must be completed. If applicants refuse to provide the required personal information, the Association will not be able to process their membership application.
3. Applicants should promptly notify the Association of any changes to their personal information.
4. Membership will only become effective after the applicant's eligibility has been verified and confirmed by email.
5. The lifetime membership fee will be waived during the first year of the Association's establishment (applications submitted on or before March 31, 2024).
